    I understand that participation in any church activity or event may be hazardous, dangerous or cause injury. In signing below I assume risk of harm or injury, which may occur as a result of participating in any church sponsored activity or event in the year of 2024, I hereby release Calvary Chapel Wichita and its officers, employees, volunteers, or agents from any liability, costs or damages resulting from this participation. I also give my consent for Calvary Chapel Wichita to seek emergency treatment if necessary, and agree to accept financial responsibility for the costs related to this emergency treatment. I have been informed and I grant Calvary Chapel Wichita and its designee's the right to use my name, likeness, image, voice, appearance and performance and to use in whole or part for promotional materials and I acknowledge that I have no interest or ownership in the the aforementioned or its copyright.
